Use our established booking page or call (309) 662-8418 to ask about appointment availability. Bring a medication list, relevant reports and your insurance card if you plan to use benefits. Think about the activities that matter to you and what has changed.
Share the full picture
Tell your clinician about previous injuries, surgeries, medical conditions, pregnancy and any new neurological symptoms. Explain what helps, what aggravates symptoms and what you have already tried. Your assessment helps determine whether care here is appropriate or a medical referral is needed.
Understand the plan before starting
Ask about the reason for each recommendation, expected benefits and limitations, risks, alternatives, cost and a point for reassessment. You can ask questions, express a preference or decline a proposed treatment.
Booking and privacy
Appointments use the practice’s existing scheduling flow. This beta does not collect health histories or payment information. Avoid sending sensitive medical details in ordinary text messages; ask the office for the appropriate secure intake process.
